Description
Seen from the posterior aspect, the left atrium dominates the base of the heart, with the right and left pulmonary veins coursing medially from the lungs to enter its posterior wall. Superior pulmonary veins lie superior to their corresponding inferior pulmonary veins, and each pair approaches the atrium on its ipsilateral side before converging on the atrial surface. The oblique pericardial sinus is implied as the recess posterior to the left atrium between the venous reflections. Coronary vessels trace the epicardial surface inferiorly toward the atrioventricular groove. Posterior pulmonary venous anatomy matters because it defines the left atrial inflow region that electrophysiologists target during catheter ablation for atrial fibrillation, where myocardial sleeves can extend onto the pulmonary veins and act as common trigger sites. Separate ostia, common trunks, and accessory pulmonary veins alter mapping strategy and influence the risk of pulmonary vein stenosis when energy delivery occurs too deep within the vein. This is the terrain. It also frames transseptal approaches, where spatial orientation to the posterior left atrium helps avoid excessive posterior wall ablation that can contribute to atrioesophageal injury.
