Centered in the upper abdomen, the pancreas spans transversely from the C loop of the duodenum toward the splenic hilum, with the broad head lying right of midline and the tail tapering leftward toward the spleen. A distinct neck crosses anterior to the portal vein confluence, and the body passes posterior to the stomach in the lesser sac, typically aligned with the level of the transpyloric plane. Superior and posterior relations commonly include the splenic artery along the pancreatic superior border and the splenic vein coursing posteriorly. Simple anatomy. Hard consequences. For teaching pancreatic anatomy, this full-length presentation clarifies why disease and intervention behave differently by region: carcinoma in the pancreatic head often compresses the intrapancreatic common bile duct and presents with painless obstructive jaundice, while lesions in the body or tail more often remain occult until late and may invade the splenic vessels.
