Description
Oriented in a posterior perspective, the pancreas occupies the retroperitoneal upper abdomen with its head nestled in the C loop of the duodenum on the right and its tail extending leftward toward the splenic hilum. The main pancreatic duct (ductus pancreaticus) courses centrally through the gland, while the common bile duct (ductus choledochus) descends on the posterior aspect of the pancreatic head toward the hepatopancreatic ampulla. Superior and slightly anterior to the proximal extrahepatic ducts sits the gallbladder (vesica biliaris), continuous with the cystic duct and common hepatic duct at the porta hepatis region. Spatial relationships read cleanly from behind. Posterior anatomy is where the trouble often declares itself. A stone impacted at the distal common bile duct can obstruct biliary drainage and, by blocking the shared outflow at the ampulla of Vater, precipitate gallstone pancreatitis with pancreatic ductal hypertension and enzyme activation. This angle also matches what you need when teaching why pancreatic head carcinoma frequently presents with painless jaundice, because the tumor compresses the intrapancreatic segment of the bile duct before it causes obvious pancreatic insufficiency.
