Description
Visible within the uterine cavity is a 16-week human conceptus in a flexed fetal posture, suspended in amniotic fluid and connected by a coiled umbilical cord to a superiorly positioned placenta. Craniofacial contours are well formed, with the head still proportionally large relative to the trunk, and the upper and lower limbs lie anterior to the torso with distinct elbows, knees, wrists, and ankles. Proximally, the cord inserts at the umbilicus and courses toward the chorionic plate of the placenta, while the surrounding decidualized uterine wall forms the outer boundary of the intrauterine space. Fingers and toes are clearly individualized. Week 16 sits in the early second trimester, a teaching sweet spot because the fetus has transitioned from the embryonic period but still retains proportions and surface anatomy that explain many downstream clinical decisions. Placental location and cord insertion matter in obstetrics: an abnormally low-lying placenta can evolve into placenta previa, and a marginal or velamentous cord insertion increases the risk of vasa previa and fetal hemorrhage if unrecognized. Dating also matters. Biometric parameters assessed around this stage, such as biparietal diameter and femur length, anchor ultrasound gestational age estimates and help interpret discordant fundal height or suspected growth restriction.
