Description
Running along the medial thigh, the gracilis is shown as a long, superficial strap muscle extending from the anterior body of the pubis and inferior pubic ramus to the proximal medial tibia at the pes anserinus. Proximally it lies inferior to the pubic symphysis and medial to the femur, then courses distally along the medial compartment, posterior to the adductor longus and anterior to the adductor magnus. Near the knee, its tendon passes posterior to the medial femoral condyle and inserts anteromedial to the tibial tuberosity, just superior to the tendons of semitendinosus and sartorius. Bony landmarks include the hemipelvis, femur, patella, tibia, fibula, and the tarsals, metatarsals, and phalanges distally. Medial views matter because gracilis is one of the few thigh adductors that crosses both the hip and knee, so it couples hip adduction with knee flexion and contributes to medial rotation of the tibia when the knee is flexed. Surgeons and therapists also track it because the gracilis tendon is routinely harvested, alone or with semitendinosus, for anterior cruciate ligament reconstruction, and its distal attachment helps orient the pes anserinus bursa, a common pain generator in anteromedial knee symptoms. Long fiber length, superficial course, predictable tendon. That is why it gets picked.
