Description
Greater wing of the sphenoid (ala major) occupies the lateral floor of the middle cranial fossa in this superior perspective, forming a broad, shallow concavity that supports the inferior surface of the temporal lobe. Medially, its boundary approaches the body of the sphenoid and the region of the superior orbital fissure, while laterally it blends toward the squamous temporal bone and the anterior margin of the temporal fossa. Within the greater wing, the foramina are positioned in a characteristic anteromedial to posterolateral sequence: foramen rotundum (opening toward the pterygopalatine fossa) lies anterior to foramen ovale, and foramen spinosum sits posterolateral, near the spine of the sphenoid and the sphenosquamous suture. Bony contours and ridges around these apertures read as fixed landmarks. No guesswork. Neurosurgical and skull base teaching often hinge on recognizing how these openings map to neurovascular pathways. Foramen rotundum transmits the maxillary nerve (CN V2), foramen ovale transmits the mandibular nerve (CN V3) with the accessory meningeal artery and emissary veins, and foramen spinosum carries the middle meningeal artery, a vessel implicated in epidural hematoma after temporal region trauma. Orientation from a superior view helps you relate the foramina to the middle cranial fossa floor and to adjacent areas where cavernous sinus pathology, perineural tumor spread, or percutaneous trigeminal procedures are discussed.
