Description
Centered on a cut section of the adult male skull, the cranial vault is opened to expose the cerebral hemispheres and the meningeal coverings, with the arachnoid mater positioned deep to the dura mater and superficial to the pia mater. The dura lines the inner table of the calvaria and reflects toward the midline, while the arachnoid bridges across the sulci, maintaining the contour of the gyri rather than dipping into cortical grooves. Fine vascular markings on the cortical surface sit in the subarachnoid space, and the pia closely invests the brain surface, tracking into the sulci inferior to the arachnoid. Emphasis on the arachnoid’s relationship to the dura and to the subarachnoid space matters because this is where pathology collects and where procedures are targeted. Subarachnoid hemorrhage tracks along the sulci and basal cisterns within the subarachnoid space, while subdural hematoma accumulates between dura and arachnoid after tearing of bridging veins that traverse from cortical veins toward the dural venous sinuses.
