Seen in lateral profile, the tibia and fibula descend to the ankle mortise and articulate with the talus, with the calcaneus forming the posterior heel and the cuboid and lateral cuneiform supporting the lateral longitudinal arch. Distally, the metatarsals and phalanges align in the forefoot, with joint cartilage indicated at the tibiotalar, subtalar, midtarsal, tarsometatarsal, and metatarsophalangeal levels. Superimposed blue vascular tracks plausibly follow the course of the dorsalis pedis artery across the dorsum toward the first intermetatarsal space and the lateral marginal vein continuing proximally as the small saphenous vein posterior to the lateral malleolus. Spatial relationships are clear, with the lateral malleolus positioned proximal and lateral to the talus, and the calcaneus posterior and inferior to the talar body. Lateral pedal vasculature matters because this is where surface anatomy, bony landmarks, and distal perfusion meet in daily practice. Palpation of the dorsalis pedis pulse is performed just lateral to the extensor hallucis longus tendon, and diminished flow is a common bedside clue in peripheral arterial disease and diabetic foot ischemia. This perspective also supports teaching the venous pathway that feeds the small saphenous system and helps orient incisions and flap planning around the lateral hindfoot and ankle, where soft-tissue coverage can be unforgiving.
