Description
Positioned at the posterior laryngeal inlet, the paired corniculate cartilages (cartilagines corniculatae) cap the apices of the arytenoid cartilages, sitting superior and slightly medial to the muscular processes and vocal processes. Around them, the thyroid cartilage lies anterior and superior, the cricoid cartilage forms an inferior ring, and the superior tracheal rings continue caudally. Intrinsic laryngeal muscles course along the posterolateral framework, and the true vocal folds extend anteromedially from the arytenoids toward the inner surface of the thyroid cartilage. Small structures. Big landmarks. These tiny hyaline cartilages matter because they define the contour of the aryepiglottic folds and contribute to the rim of the laryngeal aditus, the zone endoscopists use to orient themselves before assessing the glottis. In male larynges, the larger thyroid cartilage and longer vocal folds can dominate the field, so identifying the corniculate tips above the arytenoids helps you keep posterior glottic anatomy straight when evaluating edema, posterior commissure hypertrophy from reflux laryngitis, or post intubation granuloma near the vocal processes. For surgeons, they sit close to the interarytenoid region where mucosal incision and suturing occur during endoscopic posterior glottic procedures.
