Description
Emerging from the inferior surface of the cerebrum and the ventral aspect of the brainstem, the cranial nerves appear bilaterally in yellow against the pink brain parenchyma. Anteriorly, the olfactory bulbs and tracts (CN I) course along the orbital surface of the frontal lobes, while the optic nerves (CN II) run posteromedially toward the optic chiasm at the base of the diencephalon. More caudally, multiple nerve rootlets arise from the midbrain, pons, and medulla, with the trigeminal nerve (CN V) leaving the anterolateral pons and the facial and vestibulocochlear nerves (CN VII and CN VIII) emerging at the cerebellopontine angle, lateral to the pontomedullary junction. Symmetry is the organizing cue. Correlating each nerve to its brainstem level is central to lesion localization, and a ventral brain view makes that logic easier to teach and remember than a schematic list. A classic example is a cerebellopontine angle mass such as a vestibular schwannoma, which often affects CN VIII with early sensorineural hearing loss and tinnitus, then involves CN VII with facial weakness as the lesion expands in the internal acoustic meatus and adjacent cistern. Lower down, rootlets of CN IX, X, and XI at the postolivary sulcus frame the clinical pattern of jugular foramen syndromes, while CN XII emerges preolivary and tracks toward the hypoglossal canal, a distinction that matters when interpreting medullary strokes. Neurology and neuroanatomy courses use this plate to anchor cranial nerve numbering to real three-dimensional relationships, from the midbrain to the medulla and from medial to lateral root exit zones. It also fits well in operative anatomy and neuroradiology teaching files when introducing basal cisterns, posterior fossa approaches, and the expected cranial nerve crowding around the pons and cerebellum.
