Description
Seen through a transparent skull base and upper torso, the external vertebral venous plexus forms a longitudinal, valveless network draped over the posterior elements of the cervical vertebrae, tracking along the laminae and spinous processes from the occipital region inferiorly. Paired channels run on either side of the midline and communicate freely across the interspinous region, while segmental veins course laterally toward the transverse processes. The cranial contents remain visible, with the brain superior to the foramen magnum and the cervical spinal cord continuing caudally within the vertebral canal; peripheral nerves exit laterally in the expected cervical distribution near the intervertebral foramina. Portions of the clavicle and scapula frame the cervicothoracic junction in anterior and lateral relation. Because these veins lack valves, they provide a low-pressure conduit between pelvic, thoracic, and cranial venous territories, and they can transmit pressure changes and metastatic or infectious spread without the directionality seen in most systemic veins. A classic correlation is hematogenous dissemination of prostate carcinoma and other pelvic malignancies to the vertebrae and cranial cavity via Batson’s plexus, sometimes without prominent pulmonary involvement. This posterior cervical emphasis also supports teaching around epidural and paravertebral bleeding, where venous plexus injury can contribute to epidural hematoma and complicate posterior approaches to the cervical spine. Bleeding can be brisk.
