Description
Transparent pelvic and lumbosacral vertebral elements frame a midline venous network consistent with the external vertebral venous plexus, draped posterior to the vertebral arches and in close proximity to the lumbar and sacral spinous processes. Bilateral tributaries course longitudinally alongside the posterior elements, then communicate segmentally across the midline, while the sacrum and coccyx anchor the inferior extent of the plexus. Lateral to this venous lattice, stylized neural structures suggest the lumbosacral roots and plexuses emerging from the vertebral canal and spreading toward the pelvic sidewalls. Spatially, the veins sit posterior and superficial relative to the epidural space, yet remain contiguous with pathways that can communicate with internal vertebral venous channels through intervertebral foramina. Clinical relevance hinges on the valveless nature of the vertebral venous system and its role as a low-pressure conduit between pelvic, abdominal, and thoracic venous territories. Retrograde flow through the external vertebral venous plexus is a well-described route for metastatic spread to the vertebral column, classically from prostate carcinoma in men (Batson venous plexus concept), and it also explains why increased intra-abdominal pressure can engorge paraspinal veins and worsen bleeding during posterior spinal exposure. The close relationship to spinous processes and posterior elements also makes this anatomy worth revisiting when planning midline approaches, percutaneous instrumentation, or interpreting prominent paravertebral vessels on MRI and CT.
