Description
Arising from the body and inferior ramus of the pubis, the gracilis forms a long, straplike medial thigh muscle that runs inferiorly along the adductor compartment toward the knee. In posterior view it lies superficially on the medial thigh, just posterior to adductor longus and anterior to adductor magnus, with its distal tendon passing behind the medial femoral epicondyle to reach the anteromedial proximal tibia. Distally, the gracilis tendon converges with sartorius and semitendinosus to form the pes anserinus on the medial tibial surface. A clean medial landmark. That oblique course across both the hip and knee is exactly why the gracilis matters clinically: it contributes to hip adduction and assists knee flexion and medial rotation, so strain patterns often present as proximal groin pain with tenderness near the pubic origin. At the knee, irritation of the pes anserine bursa can mimic medial meniscal pathology, and palpation just inferior and medial to the tibial tuberosity often reproduces symptoms. The distal tendon is also a familiar graft option in reconstructive surgery, harvested with neighboring hamstring tendons for ACL reconstruction, where tendon length, trajectory, and relationship to the medial collateral ligament guide safe dissection.
