Description
Arising from the dorsal ramus of C2, the greater occipital nerve is traced superiorly from the upper cervical spine, passing between the posterior arch region of the atlas and the lamina of the axis before coursing cranially over the occipital bone toward the posterior scalp. In the midline posterior neck, it runs medial to the mastoid region and lateral to the external occipital protuberance, with smaller occipital and upper cervical nerve branches radiating laterally around the auricle. The skull, dentition, and upper vertebral column anchor the relationships, while select cranial nerves are outlined around the external ear and facial skeleton, clarifying how cervical and cranial territories meet at the occiput. Greater occipital neuralgia is the clinical reason to care about this pathway. Irritation of the nerve where it traverses the suboccipital region or pierces the posterior cervical fascia can produce lancinating pain radiating from the nuchal line to the vertex, often mistaken for migraine or cervicogenic headache. That anatomic course also explains why a greater occipital nerve block targets the nerve just inferior to the superior nuchal line, typically 2 to 3 cm lateral to the external occipital protuberance, and why landmarking must respect nearby occipital vessels in the same corridor.
