Description
Oriented in anatomical position, the liver occupies the right upper quadrant, tucked deep to the right costal margin and extending medially into the epigastrium beneath the xiphoid region. An inferior (visceral) surface perspective is implied by the emphasis on the porta hepatis, where the quadrate lobe lies anteroinferior and medial, and the caudate lobe sits posterosuperior, adjacent to the groove for the inferior vena cava. Surrounding bony landmarks include the anterior ribs and costal cartilages laterally and the thoracic vertebral column posteriorly, framing the organ within the lower rib cage. This relationship matters when you need to teach or plan access to the hepatic hilum, because the portal triad (hepatic artery proper, portal vein, and bile duct) enters and exits at the porta hepatis, a confined corridor where iatrogenic injury carries immediate consequences. Caudate lobe anatomy is also a practical point: it often hypertrophies in cirrhosis and has direct venous drainage to the inferior vena cava, a pattern that shapes both operative strategy and radiologic interpretation.
