Presented in posterior anatomical view, the superficial and deep lymphatic vessels course from the scalp and nuchal region into the occipital and posterior auricular nodes, then descend toward the posterior cervical chain along the trapezius border. Lymphatic collectors from the back and posterior thoracic wall converge toward axillary nodes in the posterior fold, while channels from the posterior abdominal wall and pelvis track medially toward the lumbar (para aortic) nodes and cisterna chyli. Superiorly, drainage funnels into the thoracic duct as it ascends in the posterior mediastinum toward the left venous angle, with the right lymphatic duct receiving the right jugular, subclavian, and bronchomediastinal trunks. A broad map, anchored to real landmarks. Posterior mapping matters when you are correlating skin, soft tissue, and deep infections of the back with likely nodal basins, or when you need to explain why a lesion near the midline can still send metastases to the axilla through posterior intercostal pathways. The thoracic duct’s course behind the esophagus and between the azygos vein and aorta is a frequent teaching point because it is vulnerable during left thoracic surgery and can present clinically as chylothorax after mediastinal or spinal procedures.
