Description

Centered in the anterior neck, the median thyrohyoid ligament appears as a thickened midline band within the thyrohyoid membrane spanning from the superior border of the thyroid cartilage to the body of the hyoid bone. Lateral to it, the thinner thyrohyoid membrane fans toward the greater horns of the hyoid, while the paired laminae of the thyroid cartilage form the prominent anterior laryngeal framework inferiorly. Inferior to the thyroid cartilage, the cricoid cartilage caps the upper airway above the first tracheal rings. Clear midline anatomy. This midline suspensory complex matters whenever the larynx is mobilized, palpated, or approached surgically, because the thyrohyoid membrane is the gateway between the suprahyoid region and the laryngeal vestibule. Clinicians target the thyrohyoid membrane for superior laryngeal nerve blocks and for laryngeal access in some emergency airways, and a midline focus helps distinguish the median thyrohyoid ligament from the more lateral region where the internal branch of the superior laryngeal nerve and superior laryngeal vessels typically pierce the membrane. Understanding its relationship to the thyroid cartilage also frames the mechanics of hyolaryngeal elevation during swallowing and the changes in laryngeal height seen with trauma or postoperative scarring.
