Description
Rendered in lateral profile, the orbicularis oris encircles the oral fissure as a sphincteric complex within the upper and lower lips, draped superficial to the maxillary and mandibular alveolar processes and wrapping anterior to the dentition. Superiorly it approaches the base of the nose and philtral region, while inferiorly it blends toward the mentum over the mandibular symphysis. At the oral commissure, decussating fibers converge at the modiolus, where slips from buccinator (posterior), zygomaticus major and levator anguli oris (superolateral), and depressor anguli oris and depressor labii inferioris (inferolateral) interlace with the perioral ring. Bone and teeth provide fixed landmarks. Perioral function depends on this fiber architecture: the orbicularis oris must seal the lips against the incisors for oral competence, then coordinate with buccinator and the elevators and depressors of the mouth for speech articulation, blowing, and bolus containment. Denervation after facial nerve (CN VII) injury, or iatrogenic weakness following parotid or submandibular surgery, produces drooling and impaired bilabial phonemes because the sphincter loses its anterior seal. Surgeons also rely on the modiolus as a stable landmark during cleft lip repair, commissuroplasty, and reconstruction after perioral trauma or tumor excision. A small muscle with big consequences.
