Framed on the retroperitoneum, the right and left renal veins course anteromedially from the renal hila to the inferior vena cava, with the right renal vein taking a shorter, more direct path and the left renal vein crossing anterior to the abdominal aorta. Superiorly, the lower lobes of the lungs and the inferior mediastinal vasculature set the cranial boundary, while the liver occupies the right upper quadrant and the spleen (lien) sits in the left hypochondrium, lateral to the left kidney. Tributaries consistent with this region, including the left suprarenal vein and left gonadal vein, are logically positioned to drain into the left renal vein, whereas their right-sided counterparts more often enter the vena cava directly. Spatial relationships are clear. Veins sit anterior to arteries at the hilum. Attention to renal venous anatomy matters because the left renal vein is a common site of entrapment between the superior mesenteric artery and aorta (nutcracker syndrome), a mechanism tied to hematuria, pelvic congestion, and left varicocele via gonadal venous hypertension. The same crossing relationship also explains why left-sided renal venous control can be more demanding during open or laparoscopic nephrectomy and why surgeons deliberately identify the adrenal and gonadal tributaries before dividing the vein. For interventionalists, this layout supports planning for renal vein sampling, IVC filter placement relative to the renal veins, and interpretation of venous phase angiography.
