Posteriorly, the male back is rendered with the superficial trapezius and latissimus dorsi opened to expose the thoracic paraspinal mass, including the erector spinae columns running longitudinally on either side of the spinous processes. Deep to these, the rotatores muscles are highlighted as short, steeply oblique slips spanning from a transverse process superomedially to the lamina and base of the spinous process one to two vertebral levels above, most developed in the thoracic spine. Costal angles and rib attachments sit lateral to the thoracic vertebrae, providing a clear lateral boundary for the transversospinalis layer. Fiber direction matters. Rotatores are often taught as minor extensors, but this view clarifies their role as segmental stabilizers and rotators that couple small axial rotation with proprioceptive feedback via dense muscle spindle content. Their deep position relative to multifidus and semispinalis makes them relevant when explaining why mid-thoracic pain can persist despite strengthening the larger erector spinae, and why targeted motor control work or injection planning must respect the layered anatomy along the laminae and transverse processes. For posterior approaches to the thoracic spine, these slips also mark the plane immediately dorsal to the facet joints and dorsal rami.
