Description
Running along the posterolateral thigh, the short head of the biceps femoris arises from the lateral lip of the linea aspera and the lateral supracondylar line of the femur, sitting deep and anterior to the long head that descends from the ischial tuberosity. Distally, both heads converge toward a shared tendon that passes posterior to the lateral femoral condyle to insert on the lateral aspect of the head of the fibula. Nearby landmarks include the posterior femur, the knee joint line, and the proximal tibia and fibula, with the muscle belly positioned lateral to semitendinosus and semimembranosus in standard anatomical relationships. Shorter by design. That contrast in length and origin matters. The short head is the only lateral hamstring component that does not cross the hip joint, so it contributes to knee flexion and external rotation of the tibia without assisting hip extension, a point that clarifies physical exam findings when comparing resisted knee flexion at different hip angles. Clinically, its distal tendon forms part of the lateral boundary of the popliteal fossa and lies close to the common fibular (peroneal) nerve as it courses around the fibular neck, a relevant consideration in posterolateral knee approaches and in traction or compression neuropathies.
