Description
Centered on the posterior thoracic vertebral column, the highlighted segment spans roughly T1 through T12, with spinous processes forming a midline ridge inferior to the cervical region and superior to the lumbar spine. Flanking each vertebra, the laminae and transverse processes sit posterolateral to the vertebral bodies, and the ribs articulate laterally at the costotransverse and costovertebral joints to build the rib cage around the thorax. Inferiorly, the thoracic spine transitions toward the thoracolumbar junction above the sacrum and pelvic ring, while the skull and appendicular skeleton provide whole-body orientation in anatomical position. Landmarks are unambiguous. Posterior thoracic anatomy matters because this is where surface and bony landmarks guide level identification for clinical care, from counting ribs to localize a suspected T7 to T10 compression fracture to planning a posterior approach for thoracic laminectomy. The steep inferior angulation of thoracic spinous processes, most pronounced in the mid-thoracic region, is a common source of level miscounts on physical exam and in the operating room unless rib attachments are used as corroborating references. It also frames teaching about thoracic kyphosis and scoliosis, where rotation and rib prominence are assessed relative to the spinous process line.
