Description
Centered in the anterior neck, the male thyroid cartilage forms the largest laryngeal framework, its paired laminae meeting in the midline to create the laryngeal prominence (Adam’s prominence). Superiorly, the epiglottis rises behind the superior thyroid notch, while inferiorly the thyroid cartilage articulates with the cricoid cartilage at the cricothyroid joints; the cricoid completes a ring and sits immediately caudal to the thyroid. Posteriorly, the arytenoid cartilages perch on the cricoid lamina, providing the attachment points for the vocal folds that span anteromedially toward the inner surface of the thyroid cartilage. Intrinsic laryngeal muscles, color-coded in red, course between these cartilages and along the vocal ligament to shape the rima glottidis. Clear landmarks. Clinical relevance concentrates at the cricothyroid membrane between the inferior border of the thyroid cartilage and the superior arch of the cricoid, the target for emergency cricothyrotomy when endotracheal intubation fails. The prominent male thyroid angle also frames key teaching points in voice physiology: contraction of the cricothyroid muscle tilts the thyroid cartilage anteroinferiorly on the cricoid, lengthening the vocal folds and raising pitch, while injury to the external branch of the superior laryngeal nerve during thyroid or carotid surgery pares this mechanism. Relationship to the arytenoids matters too, since recurrent laryngeal nerve palsy alters arytenoid position and leaves the vocal fold fixed in a paramedian posture.
