Description
Centered low in the anterior neck, the thyroid gland wraps the upper trachea with right and left lobes joined by a midline isthmus, typically spanning the second to fourth tracheal rings. Superiorly, each lobe approaches the thyroid cartilage and oblique line, while inferior poles taper toward the thoracic inlet. Laterally, the gland abuts the carotid sheath contents, with the common carotid artery and internal jugular vein flanking it, and the sternocleidomastoid and infrahyoid strap muscles forming an anterior and anterolateral muscular cover. Posterior to the gland, the cervical esophagus and prevertebral region sit deeper, and small parathyroid glands may be indicated along the posterior thyroid capsule. An anterior gross anatomy view like this earns its keep when teaching surgical planes and risk structures in thyroidectomy. Relationship to the recurrent laryngeal nerve in the tracheoesophageal groove and the superior laryngeal nerve near the superior thyroid pole guides safe ligation of the superior thyroid artery and vein, while the proximity of parathyroids and their end-arterial blood supply frames the discussion around postoperative hypocalcemia.
