Description
Deep to the external oblique and internal oblique, the transversus abdominis wraps the lateral abdominal wall with near horizontal fibers that course medially toward its broad aponeurosis. Superiorly it approaches the costal margin and lower ribs, while inferiorly its fibers arise toward the iliac crest and inguinal region, then continue into the anterior abdominal wall where the rectus sheath forms. Medially, the right and left aponeuroses meet at the linea alba, and laterally the muscle transitions to thoracolumbar fascia over the posterior abdominal wall. Clean fiber direction. Clinically, this layer matters because it contributes to the posterior wall of the inguinal canal and to the conjoint tendon, landmarks that guide open inguinal hernia repair and laparoscopic preperitoneal dissection. Transversus abdominis plane (TAP) blocks target the neurovascular plane between internal oblique and transversus abdominis, so the relationship of the muscle belly to its investing fascia and aponeurosis determines where local anesthetic spreads for anterior abdominal wall analgesia.
