Description
Humeral head anatomy is centered on the hemispherical articular surface (caput humeri) at the proximal end of the humerus, bordered peripherally by the anatomical neck where the joint capsule attaches. From an anterior viewpoint, the greater tubercle sits lateral to the head and the lesser tubercle projects more anteriorly and medially, with the intertubercular sulcus (bicipital groove) descending between them toward the proximal shaft. Inferior to the tubercles, the narrowing toward the surgical neck marks the transition to the diaphysis. Orientation matters. Clinically, these landmarks anchor the mechanics and pathology of the glenohumeral joint: the articular surface that apposes the glenoid cavity, the capsular attachment at the anatomical neck, and the tubercles that accept rotator cuff insertions (subscapularis on the lesser tubercle, supraspinatus and infraspinatus on facets of the greater tubercle). The fixed anterior perspective supports teaching and surgical planning for anterior shoulder dislocation patterns by keeping the tubercles and bicipital groove in a single frame, a helpful reference when correlating with an AP shoulder radiograph or an anterior deltopectoral approach. Proximal humerus fractures are often described relative to the surgical neck and tubercles, and this image keeps those fracture descriptors anatomically honest.
