Description
Viewed superiorly, the liver’s convex diaphragmatic surface dominates the field, with the right lobe extending laterally and the left lobe tapering toward the midline. The falciform ligament is the expected midline landmark on this surface, separating right and left hepatic territories as it courses toward the superior surface and anterior margin. Posteriorly, the bare area and the groove for the inferior vena cava would lie near the posterosuperior aspect of the right lobe, while the coronary ligament reflections define the transition between peritoneal and nonperitoneal surfaces. Parenchyma appears smooth, consistent with a healthy hepatic capsule. A superior (diaphragmatic) view matters because it fixes orientation for cross sectional imaging and operative planning when you need to relate hepatic contours to the diaphragm and the retrohepatic inferior vena cava. Small changes along this surface, such as nodularity from cirrhosis, capsular retraction from treated metastases, or subcapsular hematoma after blunt trauma, are often first appreciated as contour abnormalities rather than deep parenchymal findings. Clean surface anatomy helps you teach why hepatic segments are described in relation to hepatic veins and the IVC even when the porta hepatis is not visible.
