Description
Viewed in cross-section from the gastric lumen outward, the wall is organized into mucosa, submucosa, muscularis externa, and serosa (or adventitia where retroperitoneal). Tall simple columnar surface mucous cells line the lumen and descend into foveolae gastricae (gastric pits) that open into tubular gastric glands within the lamina propria, with a distinct muscularis mucosae forming the deep boundary of the mucosa. Beneath, the submucosa contains larger vessels, lymphatics, and the submucosal (Meissner) plexus, while the muscularis externa is arranged as inner oblique, middle circular, and outer longitudinal smooth muscle, capped by a mesothelial serosa. Rugae reflect folding of mucosa and submucosa into the lumen. This layered organization matters because most clinically relevant processes localize to specific strata: peptic ulcer disease breaches the mucosa and muscularis mucosae and, once it reaches submucosal arteries, produces brisk upper GI hemorrhage. That three-layered muscularis externa is also the mechanical basis of gastric mixing and emptying, and it frames operative and endoscopic anatomy at the pylorus, where circular muscle thickening forms the pyloric sphincter and hypertrophy produces infantile hypertrophic pyloric stenosis. Short axis histology makes depth of invasion tangible.
