Description
Seen from a superior vantage over the skull base, the hypoglossal nerve (CN XII) emerges from the preolivary sulcus of the medulla and runs anterolaterally toward the hypoglossal canal in the occipital bone. Its rootlets sit medial to the jugular foramen and posterior to the clivus, with the foramen magnum centered inferiorly as the dominant bony landmark. Nearby cranial nerves can be oriented by their exits: CN IX, X, and XI converge on the jugular foramen lateral to CN XII, while the internal acoustic meatus and petrous temporal ridges mark the posterolateral skull base. A tight corridor. This relationship matters when localizing lower cranial neuropathies. A lesion in the hypoglossal canal, such as a schwannoma, metastatic deposit, or occipital condyle fracture, can produce ipsilateral tongue weakness and atrophy with deviation toward the side of the lesion, while relative sparing of CN IX to XI points you away from the jugular foramen. Surgeons also use these bony limits during approaches to the foramen magnum and occipital condyle, where over-drilling risks postoperative dysarthria and dysphagia from CN XII injury. Use this plate in neuroanatomy teaching to anchor cranial nerve exits to named foramina, or in ENT and skull base surgery materials when discussing the hypoglossal canal, jugular foramen syndrome, and imaging interpretation on CT of the craniovertebral junction.
