Description
Running along the lateral aspect of the right lower limb, the deep crural fascia sends septa to the fibula that partition the leg into anterior, lateral, and posterior compartments. The anterior intermuscular septum lies between tibialis anterior and extensor digitorum longus anteriorly and fibularis (peroneus) longus and brevis laterally, while the posterior intermuscular septum separates the lateral compartment from soleus and the lateral head of gastrocnemius posteriorly. The fibula forms the lateral bony strut, with the tibia positioned more medial and anterior in this lateral projection; distally, the calcaneal (Achilles) tendon descends posterior to the ankle toward the calcaneus. Compartment anatomy is not an academic nicety. These septa and the interosseous membrane fix the boundaries that raise intracompartmental pressure in acute compartment syndrome, and a lateral view clarifies why pain with passive toe flexion points you toward the deep posterior compartment rather than the fibular compartment. Surgeons planning fasciotomy incisions use the fibula and the intermuscular septa as tactile guides, and this perspective also helps teach safe corridors around the superficial fibular nerve as it exits the lateral compartment in the distal third of the leg.
