Description
Spinal trigeminal nucleus (nucleus spinalis nervi trigemini), pars interpolaris, occupies the dorsolateral tegmentum at the pontomedullary junction, positioned medial to the spinal trigeminal tract (tractus spinalis nervi trigemini) and lateral to the adjacent reticular formation. From a posterior brainstem perspective, the interpolar gray appears as a longitudinal column deep to the dorsal surface, aligned rostrocaudally between the pars oralis superiorly and pars caudalis inferiorly. Nearby medullary sensory pathways provide orientation, with the dorsal column system (fasciculi and nuclei gracilis and cuneatus) situated more medially and the dorsolateral sensory bundles clustering toward the lateral margin. Spatial relationships matter here. Pars interpolaris is a practical landmark for teaching how facial pain and temperature enter the brainstem and synapse before ascending, and it helps localize deficits when symptoms do not fit a pure peripheral trigeminal distribution. Lateral medullary infarction (Wallenberg syndrome) often involves the spinal trigeminal tract and nucleus, producing ipsilateral loss of pain and temperature from the face, a bedside finding that points you toward dorsolateral medullary involvement rather than a thalamic or cortical lesion. The posterior perspective supports that localization by separating the dorsolateral trigeminal complex from the more medial dorsal column nuclei, which relate to vibration and proprioception from the body rather than facial nociception.
