Description
Running along the ulnar and radial margins of the digits, the lateral digital fibers are shown from a posterior (dorsal) perspective over the hand skeleton, tracking alongside the proximal, middle, and distal phalanges toward the nail plates. Dorsally, these collagenous bands blend with the extensor hood at the metacarpophalangeal level and continue distally as part of the lateral bands that flank each interphalangeal joint. Proximally, the carpal bones (scaphoid through hamate) and metacarpals form the rigid base from which these soft tissue restraints align, while blue articular cartilage highlights the joint surfaces at the carpometacarpal, metacarpophalangeal, and interphalangeal articulations. Orientation is clear. Dorsal-lateral fiber anatomy matters because small disruptions here change fingertip mechanics out of proportion to their size. Injury or attenuation of the lateral bands contributes to classic deformity patterns: a boutonniere deformity follows central slip failure with volar migration of the lateral bands at the proximal interphalangeal joint, while an imbalance at the distal interphalangeal joint underlies mallet finger. This posterior view also supports teaching of dorsal retinacular constraints and why dorsal surgical approaches to the phalanges and interphalangeal joints must respect the extensor apparatus to avoid postoperative extension lag and adhesions.
