Description
Across the anterior aspect of the knee, the lateral patellar retinaculum spreads as a fibrous expansion from the lateral border of the patella toward the anterolateral tibia, blending superiorly with the distal quadriceps tendon and inferiorly with the patellar ligament. Laterally, its fibers interdigitate with the iliotibial tract and the lateral patellofemoral ligament, while deeper slips merge with the joint capsule overlying the lateral femoral condyle. The patella sits anterior to the trochlear surface of the femur, with the proximal tibia and its articular cartilage shown inferiorly; the fibular head may appear posterolateral as a secondary bony landmark. Fine fascial bands form a lattice rather than a single cord. Lateral retinacular tension strongly influences patellofemoral tracking, so an anterior view is the cleanest way to relate retinacular fibers to the patellar margins and to the patellar tendon axis. Tightness or scarring in the lateral patellar retinaculum is a common contributor to lateral patellar tilt and anterior knee pain, and it is routinely assessed when planning or documenting a lateral retinacular release or lengthening. Clear depiction of its relationship to the iliotibial tract and capsule helps explain why aggressive release can produce medial patellar instability.
