Description

Oriented from the medial (mediastinal) surface, the left lung is presented with the cardiac notch indenting the anterosuperior border and the diaphragmatic surface sweeping inferiorly toward the base. At the hilum, the left main bronchus enters posteriorly, while the left pulmonary artery lies superior to the bronchus and the pulmonary veins course more anterior and inferior as they converge toward the left atrium. The superior and inferior lobes are separated by the oblique fissure, which runs from a posterior, superior origin toward the anteroinferior margin. This medial perspective matters because it maps directly to what you negotiate in thoracic surgery and bronchoscopy: the relationships at the root of the lung are asymmetric, and on the left the pulmonary artery sits above the bronchus (RALS is the memory aid, and this side is the “L”). It also clarifies why left-sided cardiac enlargement or pericardial effusion can produce adjacent atelectasis or altered breath sounds along the cardiac notch and anteromedial lung. Small space.
