Description
Positioned in the right upper quadrant beneath the right hemidiaphragm, the liver is shown from its visceral (inferior) surface with the right lobe occupying a more lateral, larger volume than the left lobe, which tapers toward the epigastrium. Anteriorly, the inferior margin would relate to the costal margin, while posteriorly the bare area approaches the inferior vena cava groove. The porta hepatis lies on the visceral surface between the caudate and quadrate lobes, where the hepatic portal vein sits posterior to the proper hepatic artery and the common hepatic duct. Gallbladder fossa and the impressions for stomach, duodenum, and right kidney are logical adjacent landmarks on this aspect. A short-range cellular term such as hepatocyte refers to the parenchyma but is not individually resolved at this scale. Visceral surface anatomy matters when you need to orient the extrahepatic biliary tree and vascular inflow, because operative exposure and anatomic variation often declare themselves at the porta hepatis and gallbladder fossa. This is the field for cholecystectomy and bile duct exploration, where misidentifying the common hepatic duct, cystic duct, or right hepatic artery can lead to bile leak or ischemic injury. Portal triad relationships and proximity to the inferior vena cava also frame hepatic resection planning and management of portal hypertension complications.
