Description
Oriented from a posterior aspect in an adult male, the liver dominates the field with the right lobe extending laterally and inferiorly relative to the smaller left lobe. The diaphragmatic surface curves superiorly toward the central tendon region, while the visceral surface turns anteroinferiorly to relate to the stomach on the left and the duodenum and head of the pancreas on the right. Near the inferior margin, the gallbladder sits in its fossa, with the extrahepatic biliary tree tracking toward the first part of the duodenum; posterior proximity to the celiac plexus and periarterial autonomic fibers is suggested by the branching green network. Posterior liver anatomy matters when you are teaching the bare area and the peritoneal reflections that tether the organ to the diaphragm and posterior abdominal wall, because those relationships explain how subphrenic collections track and why diaphragmatic irritation can refer pain to the shoulder via the phrenic nerve. The juxtaposition of the porta hepatis region with the proximal duodenum and pancreas also frames common operative and endoscopic pathways, from cholecystectomy and bile duct exploration to ERCP for choledocholithiasis with pancreatitis risk.
