Description
Oriented in anatomical position, the superior (diaphragmatic) surface of the male liver forms a smooth dome that conforms to the undersurface of the right hemidiaphragm, with the right lobe occupying a more lateral and posterior expanse than the left. Near the posterosuperior aspect, the bare area lies directly against the diaphragm where peritoneum is absent, bounded by reflections that form the coronary ligament and its right and left triangular continuations. A subtle anterior margin separates this convex surface from the inferior visceral surface that is largely out of view. Surface anatomy matters here. For operative planning, the superior liver surface gives you the clearest orientation to diaphragmatic attachments and the nonperitoneal bare area, the zone where inflammation or tumor can extend toward the diaphragm, pleura, or pericardium. This perspective also supports teaching of subphrenic and subhepatic spaces, since fluid collections can track around the liver and localize adjacent to the bare area after surgery or trauma. The same relationships explain why right shoulder pain can accompany diaphragmatic irritation (phrenic nerve referral) in hepatic abscess or hemoperitoneum. Use this artwork in gross anatomy and radiologic anatomy courses when correlating hepatic topography with coronal CT or MRI at the level of the diaphragm, or in hepatobiliary surgery and trauma publications discussing diaphragmatic lacerations, perihepatic packing, and access via right subcostal or thoracoabdominal approaches.
