Description
Palmar anatomy of the male hand is rendered with emphasis on the lumbrical muscles arising from the tendons of flexor digitorum profundus in the distal palm, then coursing distally along the radial side of digits II to V. Each lumbrical passes anterior to the metacarpophalangeal joint and swings dorsally to blend with the extensor expansion (dorsal digital hood) on the proximal phalanx, placing it functionally between the flexor and extensor systems. Superficial landmarks in this anterior perspective logically include the flexor tendons within the digital fibrous sheaths, the metacarpals and phalanges deep to the soft tissues, and portions of the carpal region and flexor retinaculum proximally. Small muscles, big consequences. Because the lumbricals link flexor digitorum profundus to the extensor mechanism, this view is a clean way to teach their signature action, metacarpophalangeal flexion with interphalangeal extension, and to explain why intrinsic muscle failure produces the classic clawing posture. Ulnar nerve palsy (digits IV and V) and combined median and ulnar neuropathy can be discussed here with specificity by tracking which lumbricals remain innervated, and by relating their loss to imbalance between long finger flexors and extensors at the MCP and IP joints. Hand surgeons also reference this anatomy when planning tendon transfers or when interpreting sagittal band and extensor hood injuries at the knuckles.
