Plantarly reflected superficial tissues expose the four lumbrical muscles as slender, fusiform slips arising from the tendons of flexor digitorum longus in the central compartment of the sole. Each lumbrical courses distally along the medial side of its corresponding digit, lying anterior (superficial) to the interossei and deep transverse metatarsal ligament but deep to the flexor digitorum brevis and plantar aponeurosis once those layers are removed. Distal insertions blend with the dorsal digital (extensor) expansions and bases of the proximal phalanges, placing the muscles immediately adjacent to the metatarsophalangeal joints and the digital neurovascular bundles in the intermetatarsal spaces. Small muscles. Tight geography. This plantar perspective matters because the lumbricals are the main intrinsic counterforce to long toe extensors at the metatarsophalangeal joints, and their line of pull explains the common pattern of MTP hyperextension with interphalangeal flexion in claw toe deformity, seen in diabetic neuropathy and after intrinsic muscle imbalance. Surgeons planning tendon balancing, metatarsal head procedures, or plantar approaches for forefoot pain benefit from a clear map of the lumbrical paths relative to the flexor tendons and the deep transverse metatarsal ligament, where intermetatarsal neuromas and synovitis are encountered.
