Beneath the plantar skin of the male foot, the four lumbricals emerge from the flexor digitorum longus (FDL) tendons in the midfoot and run distally toward the toes, lying deep to the plantar aponeurosis and superficial to the interossei within the central compartment of the sole. Each muscle belly tracks anteriorly (distally) along the medial side of its corresponding FDL tendon slip, crossing the metatarsal shafts and approaching the metatarsophalangeal joints from the plantar aspect. Proximally, the calcaneus and midtarsal region anchor the longitudinal arches, while the metatarsals and proximal phalanges define the forefoot lever that the lumbricals help fine-tune. Functionally, this plantar perspective clarifies how the lumbricals act across two joints, assisting flexion at the metatarsophalangeal joints while contributing to extension of the interphalangeal joints via the dorsal digital expansions, a coupling that becomes clinically obvious in claw toe and hammer toe deformities.
