Description
Plantar dissection of the lateral forefoot exposes the opponens digiti minimi lying deep to the plantar aponeurosis and superficial fascia, running along the lateral border from the region of the fifth metatarsal base toward the proximal phalanx of the fifth toe. Medial to it, the flexor digiti minimi brevis occupies the same lateral plantar compartment, while the more medial plantar interossei sit between the metatarsals and the adductor hallucis spans obliquely across the central sole. The fifth metatarsophalangeal joint and the phalanges of the little toe define the distal attachment area, with the calcaneus and cuboid positioned proximally in the lateral column. Deep. The relationship to the lateral plantar neurovascular bundle is implied, with branches coursing in the intermuscular plane of the deep plantar layer. Opponens digiti minimi is often forgotten because its action, drawing the fifth metatarsal into plantarflexion and slight rotation to cup the lateral forefoot, is subtle but clinically relevant when the lateral column becomes unstable or painful.
