Description
Running along the lateral border of the plantar foot, the abductor digiti minimi is seen immediately deep to the skin and superficial fascia, forming a muscular ridge over the lateral longitudinal arch. Its proximal fibers arise from the medial and lateral processes of the calcaneal tuberosity and adjacent plantar aponeurosis, then course distally toward the base of the proximal phalanx of the fifth toe. Medial to it, the plantar aponeurosis would occupy the central sole, while the calcaneus sits proximally and the fifth metatarsal lies anterolateral in line with the muscle belly and tendon. Small cutaneous branches and the neurovascular plane for the lateral plantar nerve and artery are expected to pass deep to the first-layer musculature in this region. A plantar, subcutaneous presentation matters because the abductor digiti minimi is the first-layer landmark for orienting the lateral sole during dissection and during ultrasound-guided assessment of lateral plantar pain. Hypertrophy, edema, or focal denervation of this muscle can accompany Baxter neuropathy (entrapment of the first branch of the lateral plantar nerve) and may mimic or coexist with plantar fasciitis, a distinction that hinges on knowing what sits lateral versus medial to the plantar aponeurosis. The muscle’s relationship to the calcaneal tuberosity and lateral plantar neurovascular bundle also frames surgical corridors for plantar fascia release and lateral hindfoot approaches.
