The muscle belly originates from the lateral process of the calcaneal tuberosity and adjacent plantar aponeurosis, then runs anteriorly and slightly laterally toward the fifth ray, lying superficial to the quadratus plantae and lateral to the flexor digitorum brevis. Distally, its tendon tracks to the lateral side of the base of the proximal phalanx of the fifth toe and blends with the dorsal digital expansion. The lateral plantar neurovascular bundle typically courses deep to this first-layer muscle, a practical relationship when teaching the layered anatomy of the foot. For clinicians, isolating the abductor digiti minimi in plantar view helps clarify why heel pain is not always plantar fasciitis: the first branch of the lateral plantar nerve (Baxter nerve) often passes between the abductor digiti minimi and the medial calcaneal tubercle region and can be compressed near the muscle’s proximal fascia.
