Description
Seen from the lateral aspect of the male foot and ankle, the abductor digiti minimi occupies the most lateral layer of the plantar foot, arising from the lateral process of the calcaneal tuberosity and the adjacent plantar aponeurosis. Its fleshy belly courses anteriorly along the lateral plantar margin, superficial to the quadratus plantae and in close relationship to the tendon of flexor digitorum longus as those deeper structures pass toward the forefoot. Distally, the muscle narrows toward the base of the proximal phalanx of the fifth toe, lateral to the shafts of the fifth metatarsal and the metatarsophalangeal joint. A lateral view matters because it clarifies how the lateral plantar musculature supports the longitudinal arch while countering inversion moments at the subtalar joint during stance and push-off. This is a frequent pain generator in lateral heel and midfoot complaints, where entrapment of the inferior calcaneal nerve (Baxter nerve) near the calcaneal tuberosity and abductor digiti minimi can mimic plantar fasciitis. The same regional anatomy guides surgical planning around the lateral calcaneus and helps orient clinicians during palpation and ultrasound evaluation of plantar heel pain.
