Description
Plantar skin has been rendered translucent to expose the lateral compartment of the sole, with the opponens digiti minimi lying deep along the lateral border of the foot. Its fleshy belly sits inferior to the shaft of the fifth metatarsal and runs anteromedially from the region of the cuboid and long plantar ligament toward the proximal phalanx of the fifth toe. Superficial to it, the abductor digiti minimi and flexor digiti minimi brevis form the more superficial strata, while the plantar aponeurosis and fibroadipose heel pad remain as the outer covering. A small field, but dense anatomy. Functionally, this muscle (often treated as a variant or deep portion of the flexor digiti minimi brevis in some texts) helps draw the fifth metatarsal into plantarflexion and adduction, supporting the lateral longitudinal arch during push-off and resisting excessive splaying of the forefoot. That matters in bunionette deformity, lateral forefoot pain, and post-traumatic or post-surgical weakness after fifth metatarsal fixation, where preserving the lateral plantar neurovascular supply and the integrity of the lateral plantar fascia can affect recovery. The lateral plantar nerve, coursing medial to the abductor digiti minimi and then deep across the sole, is the structure you worry about during lateral plantar incisions and decompressions. Small muscle. Common pitfall. Use this plantar under-skin view for teaching the layered dissection of the foot in gross anatomy and podiatric anatomy courses, and for illustrating lateral column procedures in orthopaedic foot and ankle or podiatric surgery publications (bunionette correction, fifth metatarsal osteotomy, lateral plantar nerve release).
