Semi-transparent skin reveals the deep plantar compartment of the male foot, where the three plantar interossei lie against the medial aspects of metatarsals III to V. Each unipennate muscle originates from its metatarsal shaft and runs distally within the intermetatarsal spaces, passing plantar to the metatarsophalangeal joints to blend with the extensor expansion and the bases of the proximal phalanges. From this lateral perspective, the interossei sit plantar to the metatarsals and dorsal interossei, deep to the flexor tendons and plantar aponeurosis, while articular cartilage highlights the tarsometatarsal and metatarsophalangeal joint surfaces. Fine tendons are visible. Functionally, the plantar interossei provide toe adduction toward the second ray and assist metatarsophalangeal flexion with interphalangeal extension via the extensor hood, a balance often lost in intrinsic muscle wasting from diabetic neuropathy or longstanding cavus deformity. Their location between metatarsals makes them relevant when teaching intermetatarsal pain patterns, since edema, stress injury, or adjacent interdigital nerve irritation can mimic primary metatarsalgia and complicate the differential for Morton neuroma. Surgeons working around the metatarsal shafts and heads, including dorsal or plantar approaches for metatarsal osteotomy or lesser toe procedures, benefit from a clear mental map of these muscle bellies, their distal insertions, and their close relationship to the deep plantar arterial arch and lateral plantar nerve branches.
