Plantar dissection of the male forefoot exposes the three plantar interossei nestled in the 3rd, 4th, and 5th intermetatarsal spaces, arising from the medial shafts of metatarsals III to V and running distally toward the bases of the proximal phalanges and extensor expansions. Medial to lateral, the hallucal ray is dominated by the first metatarsal and proximal phalanx, while the central rays frame the interosseous compartment between adjacent metatarsals. Flexor digitorum longus tendons course distally on the plantar aspect toward the lesser toes, with lumbricals typically seen along their medial sides, and the deep transverse metatarsal ligament spans across the metatarsal heads just distal to the muscle bellies. Small but specific anatomy. Functionally, the plantar interossei adduct toes 3 to 5 toward the second ray and assist metatarsophalangeal flexion with interphalangeal extension via the extensor hood, a relationship that becomes obvious when you track each muscle’s distal slip relative to the plantar plate and the long flexor tendons.
