Description
Anterior pelvic lymphatics are rendered in an x-ray style over the bony pelvis, with lymphatic vessels converging on the external iliac, internal iliac (hypogastric), and common iliac nodal chains along the pelvic brim. Medially, collectors course toward the obturator region adjacent to the superior pubic ramus, while more lateral channels track with the iliac vessels toward the anterior superior iliac spine. Superior continuation into the lumbar (para-aortic) pathway is implied by the proximal common iliac drainage. Skeletal landmarks include the ilium, ischium, pubis, acetabula, proximal femora, and the sacrum in the background. Pelvic nodal anatomy is where staging and surgical planning become concrete. External iliac and obturator nodes form the dominant basin for prostate and bladder lymphatic spread, and they are the primary targets in pelvic lymph node dissection and sentinel node mapping, where missed nodes can sit just medial to the external iliac vein or within the obturator fossa. This anterior perspective also supports teaching patterns of metastatic disease from testes and distal lower limb by contrasting superficial inguinal drainage (not emphasized here) with deeper iliac pathways. A key landmark.
