Description
Arising from the medial cord of the brachial plexus in the axilla, the medial brachial cutaneous nerve (medial cutaneous nerve of arm) descends along the medial aspect of the proximal arm, positioned posterior to the pectoral region and anterior to the scapular plane in this posterior view. The clavicle sits superior and anterior, while the scapula lies posterolateral to the thoracic cage, framing the shoulder girdle behind the glenohumeral region and proximal humerus. Course relationships in the axilla are suggested by accompanying plexus branches, with the highlighted nerve tracking inferiorly toward the medial arm skin territory. Bony landmarks anchor orientation. Posterior visualization of this sensory nerve matters because it forces attention to where the brachial plexus branches wrap from the axilla into the arm and where iatrogenic injury can occur without motor deficit to signal it. Axillary lymph node dissection, mastectomy with axillary clearance, and even aggressive retraction during shoulder and proximal humerus procedures can stretch or transect small cutaneous branches, producing medial arm dysesthesia that patients describe as burning or numbness. Pain without weakness is the clue. In regional anesthesia, distinguishing medial brachial cutaneous involvement from intercostobrachial (T2) sensory loss helps explain incomplete coverage after an axillary block.
