Description

Oriented on the medial aspect of the male knee, the femur sits superior to the tibia with the medial femoral condyle articulating over the medial tibial plateau, while the fibular head remains only partially visible laterally. Along the medial joint line, the medial collateral ligament (tibial collateral ligament) spans from the medial epicondyle of the femur to the proximal medial tibia, with its deep fibers blending with the fibrous joint capsule. A firm attachment to the medial meniscus is emphasized, placing the ligament immediately superficial to the meniscocapsular junction. Deep in the intercondylar region, the anterior cruciate ligament and posterior cruciate ligament are positioned centrally as intra-articular, extrasynovial stabilizers, but the medial collateral ligament is the key restraint to valgus stress and contributes to control of tibial external rotation, especially in knee flexion. That meniscal attachment matters clinically. Valgus trauma can injure the superficial MCL alone or propagate into the deep MCL and meniscocapsular complex, a pattern that explains medial joint line tenderness and the classic association between MCL sprain and medial meniscal pathology, including peripheral meniscocapsular separation. Teaching knee biomechanics is easier with this medial perspective, where you can trace the superficial and deep components of the MCL relative to the capsule and meniscus while still orienting learners with the cruciate ligaments inside the notch.
